The Ministry of Education requires all elementary students to fill out a hard copy of a COVID-19 screening declaration upon their return to school after the April Break. 

Hard copies of this declaration are being sent home this week with the elementary school children who reside with you. 

For anyone who may not have received a copy of the form, we are making it available here.

Please be sure to fill out the form and send it with your child(ren) when they return to school on April 19. Elementary students will not be allowed to return to school after the break if they have not completed these COVID-19 declaration forms.
